Force browser to reload index.htm

OK, apparently no-cache was not enough.
The following does the trick:

  <meta http-equiv="cache-control" content="max-age=0" />
  <meta http-equiv="expires" content="0" />
  <meta http-equiv="expires" content="Tue, 01 Jan 1980 1:00:00 GMT" />
  <meta http-equiv="pragma" content="no-cache" />
